Please Read Before Booking
New Patients: All first time visitors are required to select a ‘New Patient’ visit before being eligible for ‘Established Patient’ or other specialty visits. A $45 deposit is required to reserve your appointment. This amount will be applied toward your visit at the time of service.
Insurance: We are currently only in-network with Blue Cross Blue Shield. If you have another insurance provider, we can provide a detailed receipt (superbill) for you to submit for possible reimbursement or application toward your deductible (varies by policy; not guaranteed). We are not participating providers with Medicare or Medicaid.
Cancellations & No-Shows Policy: Appointments must be canceled or rescheduled at least 24 hours in advance through your booking portal or by giving our office a call. Late cancellations or no-shows will result in a fee. This policy helps us respect and value everyone’s time, thank you for your understanding!
